To help us process your IVF grant application you should make sure you have all the information needed as stated below.

If your application is incomplete, unpaid or if you do not provide the information requested then you would not be eligible for a grant.

You will also be able to Save and Continue the form if you need to get further information.

APPLICATIONS CLOSE ON THE 31st March 2024

If we require more information we will contact you and if necessary which may include a video call to discuss their application.

You will be notified via email by the end of April of the outcome of your application

It should also be noted we cannot provide feedback on applications

We hope to complete the application process by May 2025.

You can fill out the application even if you do not have all the documentation to hand.

However, if you have not uploaded these within 30 days your application will be removed

Documentation check list – You will need these to complete your application

Last 3 months of bank statements – both applicants if applicable

Last 3 payslips if working – both applicants if applicable

Proof of any benefits you may receive – both applicants if applicable

Proof of funds to cover rest of the treatment, any tests you require and medication

Personal statement – both applicants if applicable

Medical history including any relevant GP or clinic notes from any treatment you have had

Female and Male fertility tests including treatment plan and costs from your fertility clinic

Eligibility check list – Please confirm you understand and have the information required.

Heterosexual, same sex couples and single women are eligible to apply

Female must be 42 and under when making the application

Couple to be British citizens or have Indefinite Leave to Remain with no criminal record and live full time in the UK

Couple to be cohabiting, with at least one half of the couple to be in full-time gainful employment at the time of application

Proof of funds to cover the rest of the treatment (if needed) including fertility medication costs

Our grants can used for IVF treatment and we do not cover fertility medication

There is a £20 NON-REFUNDABLE application fee is required, and is considered a donation which may be tax deductible – Payable on completion of application

We do not cover medical revisions or just egg freezing
